The Health Improvement Navigation Service was developed to offer personalised, one-on-one guidance, to ensure people receive the right care at the right time.
It’s no secret that the current health system is often siloed and lacks connection between different medical disciplines. Our Health Navigators provide guidance through the health system, walking alongside people to simplify the complexities and connect them to the right resources at the right time. They are trained in rehabilitation, law, public health and community engagement, and are equipped to handle a wide range of needs.

Initial assessment - a 60-minute telehealth triage assessment to understand the employee's needs. You will receive a personalised plan outlining key actions and timelines.

Follow-up - 2x 60-minute follow-up sessions to ensure the employee is progressing toward and receiving the necessary support to achieve their goals. Additional sessions are available if required.

Billing – all sessions are billed via your regular process at a rate of $198.45+gst per session.

Manager reporting - these are personal social health issues, due to privacy issues there is no reporting back to managers. A confirmation of attendance can be given if required.
